These Terms apply to all of your activities engaged in with Company through its websites and Services. By utilizing Company's website and Services, you are agreeing to these Terms including binding arbitration and class action waiver.
Your Account
You may need to set up an account with Company. You are responsible for your account and must ensure all information you provide is accurate. Hold your username and password in strict confidence. Company reserves the right to disable accounts that are compromised.
Acknowledgement Regarding Services
Company does not provide medical care, does not practice medicine, does not employ or control clinicians, and does not make medical or clinical decisions. All medical services are provided solely by independent, physician-owned medical practices and their licensed clinicians. Your patient relationship is solely with that Practice.
Accuracy of Information
Information on this website is provided for general informational purposes only and should not be relied upon as medical, legal, or professional advice.
Payment for Services
You agree to pay the fees for Services you elect to purchase and authorize Company to charge your payment method. Refunds are governed by Company's Refund Policy available at refund-policy.html.
Compounded & Prescription Medications
Any compounded medications referenced on this website or made available through an affiliated Practice are not FDA-approved. They are prepared by licensed compounding pharmacies in accordance with applicable state and federal regulations. All prescription medications require a valid prescription from a licensed clinician.
Company Rights
All right, title, and interest in Company's Services, website, applications, and databases remain the exclusive property of Company. You may not use Company trademarks, logos, or marketing materials without permission.
Age Requirement
Company does not knowingly provide Services to individuals under 18 years of age. If you are under the age of majority, you must not use the Services.

All disputes shall be settled by binding arbitration administered by the American Arbitration Association. No arbitration shall proceed as a class action. THE PARTIES WAIVE THE RIGHT TO PARTICIPATE IN A CLASS ACTION.

Company may update these Terms at any time. If Company makes any material change, it will notify you by email or website notice. Your continued use of our Services after changes become effective constitutes acceptance of those changes.
